Are you an RN who thrives on clinical decision-making, patient communication, and helping people feel safe and supported—without bedside demands? This hybrid Telephone Triage Nurse role offers the best of both worlds: meaningful team connection onsite and the flexibility of remote work.

As our Primary Care Telephone Triage Nurse , you’ll be the first clinical voice patients hear when they reach out by phone or portal. Your expert assessment and judgment will guide patients to the right level of care—keeping our community healthy, safe, and connected to the services they need.

(Current NH RN License in good standing)

• Serve as the first point of clinical contact via phone and patient portal

• Conduct thorough telephone assessments using evidence-based triage protocols

• Prioritize and direct patients to the appropriate level of care

• Provide clinical advice, education, and emotional reassurance

• Collaborate with providers, nurses, and multidisciplinary teams

• Recognize and escalate urgent or emergent situations

• Document accurately and promptly in the EHR

• Participate in quality improvement and ongoing training

• 3 days in-person at Goodwin Community Health (Somersworth, NH)

• 2 days remote from home

• Collaborate with both GSCH primary care sites (Somersworth & Portsmouth)

• Make a real impact through skilled assessment and communication

• Use your RN expertise in a focused, patient-centered environment

• Support patients of all ages, especially those facing barriers to care

• Registered Nurse (RN) – REQUIRED

• Experience in primary care, ambulatory care, or telephone triage preferred

• Current BLS certification

• Excellent clinical judgment and communication skills

• Ability to work both independently and as part of a team

• Comfort with EHR systems and patient portal communication

• $5,000 Sign-On Bonus
