As our Primary Care Telephone Triage Nurse , you’ll be the first clinical voice patients hear when they reach out by phone or portal. Your expert assessment and judgment will guide patients to the right level of care—keeping our community healthy, safe, and connected to the services they need.
✅ REGISTERED NURSE REQUIRED(Current NH RN License in good standing)
???? What You’ll DoServe as the first point of clinical contact via phone and patient portal
Conduct thorough telephone assessments using evidence-based triage protocols
Prioritize and direct patients to the appropriate level of care
Provide clinical advice, education, and emotional reassurance
Collaborate with providers, nurses, and multidisciplinary teams
Recognize and escalate urgent or emergent situations
Document accurately and promptly in the EHR
Participate in quality improvement and ongoing training
3 days in-person at Goodwin Community Health (Somersworth, NH)
2 days remote from home
Collaborate with both GSCH primary care sites (Somersworth & Portsmouth)

Support patients of all ages, especially those facing barriers to care
Registered Nurse (RN) – REQUIRED
Experience in primary care, ambulatory care, or telephone triage preferred
Current BLS certification
Excellent clinical judgment and communication skills
Ability to work both independently and as part of a team
Comfort with EHR systems and patient portal communication
$5,000 Sign-On Bonus
